Use this request form if you’re an external client interested in booking a corporate meeting, community event, or private function.
Use this request form if you are a CSU faculty or staff member planning an academic or university-related event.
CSU student orgs can follow this link to jump to VikesConnect to request bookings for student events and activities on campus.

Complete this form for all performance and entertainment bookings (required).
Complete this request to modify or cancel an existing booking or reservation. Request MUST be submitted 5 working days before event to request a change and 3 working days before event to cancel.
Responsible parties will be held financially accountable for all charges that would have been incurred because of commitments made to facilitate the event if proper notice is not given.
